INSURANCE, FEES AND PAYMENTS

In order for us to identify your file and any changed information please present your insurance card at each dental appointment!

You may have an insurance benefit payable in full or in part for services you receive. We are pleased to assist you with identifying your coverage and, in many cases, will submit a pre-authorization form to your insurance carrier. Information received from this will detail your available benefit. In all cases you are responsible for the payment of all charges not covered by your insurance plan. If an overpayment should result, a refund will be sent to you or your insurance company. We are pleased to assist you with all insurance form submission.

FEES FOR SERVICE AND PAYMENTS

After identifying the service needed you will be advised. To control costs we ask that you pay for service at the time of your visit. Where laboratory costs and orthodontic appliance cost are involved, we require a partial payment at the start of treatment. All payment arrangements and authorizations will be presented to you at times before or at the time of your appointment. Prompt payment is expected in order to avoid added costs. We are pleased to assist you in all ways to facilitate the payment of your account. Open communication is vital in order to avoid misunderstanding.
